Dentofacial esthetics flow from the outside-in. The macro-esthetic features (craniofacial) directly influence the mini-esthetic (smile) and micro-esthetic (teeth) traits. In this regard a proper diagnosis and treatment plan should first prioritize the facial features to determine what is ideal, what is deficient, what needs to be protected, and what can be improved. We all know that there is nothing inherently wrong with extracting permanent teeth to achieve a beautiful orthodontic result if it is absolutely necessary. However, this decision should be based on more variables than just math (a quantified amount of crowding or cephalometric numbers). In dental school, Dr. Foster placed in the 99th percentile on the National Board of Dental Exam, #1 in his class at USC. During his orthodontic residency, Dr. Foster was awarded "Top Clinician" (Harry L. Dougherty Award) at USC. Dr. Foster is ABO board certified: the highest level of distinction for an orthodontist (less than 1/3 of orthodontists are board certified). He is a former part-time faculty member at USC Dental School. He currently serves as Editor-in-chief for Orthotown Magazine (one of the largest orthodontic publications in the world). He is a national lecturer on research and development with Rocky Mountain Orthodontics.

The interdisciplinary management of anterior dents-facial aesthetics in the modern era has become the mainstay of modern dental practice within most dental specialties. The subjectivity of aesthetic judgements and differences between professional and lay people regarding the most commonly shared "standards of beauty" – as well as their constant fluctuation through time – have pushed clinicians to attempt to clarify and standardize these methods of assessment and treatment planning so that we can more easily and consistently communicate, plan and apply existing and future treatment techniques for the benefit of our patients.

This presentation is aimed towards orthodontists as well as other dental practitioners interested in interdisciplinary management of dento-facial aesthetic problems, where the most modern approaches to interdisciplinary management of anterior dental aesthetics will be discussed, focusing on Macro, Mini, and Micro Aesthetics.

It will demonstrate the importance of "holistic thinking" in dentistry, and showcase how orthodontic treatment can be an important foundational block for successful aesthetic treatments in most cases. It also aims to clarify the current standards of assessment, treatment planning and care required by practitioners working together in a multi-disciplinary setting, so that they would better be able to communicate, think and act harmoniously during the various stages of treatment, particularly where orthodontic treatment is required.

Dr. Shadi Samawi BDS, MMedSci (Orth), MOrthRCSED is a Specialist Orthodontist and a Founding Partner/Owner of Samawi Dental Orthodontic Center, the leading Specialty Orthodontic Center in West Amman, Jordan since 1999. He is also a former Consultant Orthodontist in Manama, The Kingdom of Bahrain, He is currently the Secretary General and Treasurer of the Jordanian Orthodontic Society and the Jordanian Commission for Orofacial Pain and Dysfunction (JCOPD).

He received his Bachelor of Dental Surgery (BDS) from the University of Jordan in Amman in 1999. Following a one-year Residency at the Royal Medical Services in Jordan and two years in private practice, he left to the UK and obtained his Master's degree in Clinical Orthodontics (MMedSci) from the University of Sheffield in 2003. He later worked as an Honorary Clinical Fellow in Orthodontics at the Charles Clifford Dental Hospital in Sheffield until obtaining the Membership in Orthodontics from the Roxal College of Surgeons of Edinburgh (MOrthRCSED) in 2004.

In this lecture, I will explore orthodontic cases that went off course due to planning or mechanical failures, patient issues, or as cases taken over from other orthodontists. Drawing from the most personal experiences, I will present "worst-case scenarios" encountered in my career.

These cases range from severe malocclusions worsened by poor planning to complications from mechanical failures or patient non-compliance. However, the focus will be on redemption, outlining strategies used to salvage these situations and get them back on track. Begin with the end in mind is one of the most important philosophies in Pitts 21 protocol. Initiating the wanted forces and moment within the appliance as early in treatment as possible, using SAP bracket position to adjust vertical position of the incisors for smile arc, inverting groups of brackets (flipped and flocked) as needed to activate torsion in the appliance sooner in treatment ,using ESLE to control forces and moments with appropriate disarticulation to encourage specific tooth movements .This has become known as an «ACTIVE EARLY» approach to case management in order to create new smiles with simple mechanics while being efficient.

Doctor Luis Ilzarbe will share his expertise in using Smile Architect TM tools included in the Clincheck software from Align, showing a multidisciplinary approach with the Invisalign system, alignment teeth prior to the restorative. Derek Mahony is a world renowned Specialist Orthodontist who has spoken to thousands of practitioners about the benefits of interceptive orthodontic treatment. Early in his career Dr Mahony learned from leading clinicians the dramatic effect functional appliance therapy can afford patients in orthodontic treatment. He has been combining the fixed and functional appliance approach ever since. His lectures are based on the positive impact such a combined treatment approach has had on his orthodontic results and the benefits this philosophy provides from a practice management viewpoint. After completing his Dental Degree at the University of Sydney Dr Mahony proceeded to the United Kingdom where he completed his Masters Degree in Orthodontics at the Eastman Dental Hospital, Institute of Dental Surgery, London. Further studies led to the successful completion of a Diploma in Orthodontics at the Royal College of Surgeons, Edinburgh. Dr. Mahony has also passed the Royal College of Dentists in Canada post graduate examination in the field of orthodontics. Dr Mahony has passed examinations leading to a postgraduate qualification in Dentofacial Orthopaedics from the Royal College of Physicians and Surgeons in Glasgow. He has also attained his Membership in Orthodontics qualification from the Royal College of Surgeons, England.

Dr Mahony has been seeing an average of 250 patients per week for the last decade and has gained a vast amount of experience which he can pass on to clinicians so that they can come to appreciate the key elements of his combined treatment approach. He currently has over 3000 orthodontic patients in active treatment and has been a key note speaker at the International Orthodontic Summit meetings, the International Association of Orthodontics meetings, and the American Association of Functional Orthodontics meetings. He is considered by some to be the "next leading lecturer on functional/fixed orthodontics."

Dr Mahony approaches his orthodontic diagnosis from a "facial profile" point of view. Luis has been qualified from School of dentistry at Public University of Valencia (Spain) in 2007.
He got his master degree in periodontology (under doctor Caffesse three years program) in CG Formación Continuada, Elche, Spain. He obtained his DUI in Implantology at Paul Sabatier University in Toulousse, France after a 2 year partial program in ESORIB. Luis has focused his training and practice since 2012 through aesthetics and minimally invasive prosthesis with stays with well-known doctors as Hirata, Fahl, Kina, Escopin, Kano, Autran, Cofar, Berbis, Stumpf and others improving his skills over direct and indirect restorations, smile design protocols and all digital workflows.

Luis is a part time professor at the Catholic University of Valencia (Spain) since 2010, as associate professor in the prosthesis department. He is the director of the master's degree in adhesive esthetic dentistry at Tech University, Las Palmas de Gran Canaria, Spain. As consequence of this activity he leads different investigation lines which result in several articles in Pubmed and become a KOL of companies like Graphenano dental and a Faculty member for Align and Aligner Dental Academy.

He has a big background lecturing for different companies in his mother country ( Straumann, 3shape, DentsplySirona and Aligntech) about Digital and Minimally Invasive Dentistry and like an international speaker for Align sharing his digital expertise with iTero, Exocad and Invisalign Clear Aligners. Dr. Amina Ben Salem Saied is a graduate of the Faculty of Dental Medicine in Monastir, Tunisia, holding a state diploma in Orthodontics. She has worked as a resident at hospitals in Lyon, France. A renowned speaker, she has presented at numerous national and international conferences in the USA, Dubai, Lebanon, Egypt, Morocco, Bucharest, Budapest, and more. Dr. AMINA BEN SALEM SAIEDS holds additional certifications in Medical Imaging (Tunisia, 2019) and Management and Clinical Efficiency (California, 2021). She completed a Fellowship in Aligner USA in 2021 and earned a Master in Finishing Pitts 21 (Tunisia )in 2022. Her qualifications also include a supplementary certificate in Aesthetic Orthodontics in Tunisia (2023).
Since 2013, she has been a member of the American Association of Orthodontics (AAO) and the World Federation of Orthodontics (WFO).
